21st Century High School Programs Interest form

We are glad you are interested in a 21st Century High School Program. We would like you to complete this interest form to help us get to know you. This form will serve as your district transfer request upon enrollment in the program.

Here's how it works:

1. Complete this two page online Student Interest Form (2 pages), including the Personal Statement of Interest Narrative and Self-Assessment form.
2. Submit the form online. You can also submit your interest form in person to your school counselor.
3. You will receive an e-mail acknowledgement of receipt of your online form.
4. You will receive an acknowledgement letter sent to your home with information for you to follow regarding the next steps in the program enrollment process.
